Use this form to tell us about a potential research collaboration, publication, recruitment need, natural history project, or patient-partnership request. Submission starts a review conversation; it does not approve data access or guarantee participation. If you already know the intended output, name it here so the review can match the right pathway, whether that is a manuscript, abstract, poster, milestone note, or plain-language summary.

Before you submit

A good application makes the consent path visible.

Be specific about the mitochondrial disease population, research question, and intended output.

Describe the minimum data or cohort support needed rather than requesting broad access.

Note whether the project has funding, IRB status, a sponsor, a planned manuscript, or a protocol timeline.

If applying independently, disclose employment, consulting, funding, outside-work, confidentiality, and invention-assignment obligations. MDA may require a waiver if another party could claim the work.

Explain how findings could be returned to the patient community in plain language.

Why cohort readiness matters

More complete follow-up makes review more useful.

Researchers and partners get a better answer when the cohort has complete basics and enough follow-up over time. That helps MDA judge whether a study idea is worth review without moving detailed records or private matching out of Mito Map.

Data completeness tells us whether the core fields are filled in.

Longitudinal depth tells us whether updates keep arriving over time.

Governed access means every request is reviewed before any data leaves Mito Map.

The MDA website shares public information; Mito Map keeps the detailed records, consent controls, and matching.
