Suggested clinic workflow
- Hand the patient the printable handout during the visit.
- Use this note after the visit if a simple portal message is easier than repeating the explanation.
- Keep the message to one sentence and one next step whenever possible.
- Route detailed records, genetics, and urgent questions through normal care channels, not the public form.

When to use this
